Carrie Hollkamp, RN,

Family Nurse Practitioner, Carrie Hollkamp has Bachelor of
Science degrees in both Education and Nursing from Indiana
University Southeast, and a Master of Science in Nursing from IUPUI. She has been part of the Clark Memorial team since 1994,
serving as a Medical-Surgical, ICU and Emergency Registered
Nurse. |

On your first visit to our
practice, please bring all prescription drugs you are currently taking,
including herbs and vitamins. If you have any drug allergies, it is
important to bring a list. Also, please bring any records from previous
physicians. This will eliminate any unnecessary delays or repetition of

A
family practice provides complete, ongoing health care for you and your
