Carrie Hollkamp, RN, FNP-C

As a Certified Family Nurse Practitioner, Carrie Hollkamp has Bachelor of Science degrees in both Education and Nursing from Indiana University Southeast, and a Master of Science in Nursing from IUPUI. She has been part of the Clark Memorial team since 1994, serving as a Medical-Surgical, ICU and Emergency Registered Nurse.

On your first visit to our practice, please bring all prescription drugs you are currently taking, including herbs and vitamins. If you have any drug allergies, it is important to bring a list. Also, please bring any records from previous physicians. This will eliminate any unnecessary delays or repetition of tests.
