Nursing Services That Meet Your Special Needs At Home
Care is individualized to meet your needs through our team approach which
includes active participation by you, your family, your doctor, and Southern Home Care
staff.
Such care could include:
- IV Therapy
- Chemotherapy
- Injections
- Venipuncture for lab work
- Catheter care
- Post-op wound care
- HIV/AIDS care
- Education
Cardio-Pulmonary Team
Our experienced nurses have years of critical care experience.
The team focuses on helping you when you have:
- Congestive heart failure
- Chronic obstructive pulmonary disease
- Pre- and post-op care for coronary artery bypass surgery,
angioplasty and heart transplant
- Oxygen therapy
- Ventilators
- Asthma
The Mother-Baby Service Program
Nursing care, education and support are provided to you in your home by our
experienced nurses.
Wound-Ostomy-Continence Home Program
The Southern Home Care WOC Program, coordinated by a
certified enterostomal therapy nurse, provides care, education and support for the
following:
- Urostomy, ileostomy and colostomy care
- Complicated and/or difficult to heal wounds
- Diabetic ulcers
- Continence re-training
- Instruction in prevention of skin breakdown
- Assistance in sizing and obtaining ostomy supplies
Home Team Rehabilitation Services
The Home Team can evaluate your needs and assign qualified
therapists to provide care. Conditions covered include:
- Post-CVA (stroke) care
- Post total knee or hip surgery
- Closed head injuries
- Injuries to bone and/or muscle
- Cardiac Rehabilitation
Here's What We Do:
Physical Therapy
Helps to increase your movement, strength, balance and coordination. We help you with
walking and movement exercises through a home exercise program.
Occupational Therapy
Helps to increase your ability to perform daily activities. Instruction is provided
in homemaking activities, meal preparation and personal care tasks. Adaptive equipment is
suggested when needed, and home safety is
evaluated.
Speech-Language Pathology
Helps you regain language and speech skills. Instruction is given to both you and
your family in alternative communication methods such as word/letter boards. Evaluation
and treatment of thought processes and swallowing disorders are also performed.
Support Services
Sometimes you may need more than nursing or therapy services. Southern Home Care
provides registered dietitians and certified social workers to complete the circle of
caring.
- Social Services: To assist you and your family with
short term counseling, if you need community resource referrals, including help in
obtaining food, medications, housing, financial aid, or long-term planning.
- Dietitians: Assess specific nutritional needs and
provide nutritional education and counseling.
- Home Health Aides: Our certified home health aides can
provide you with additional support by assisting with:
- Activities of daily living
- Personal care
- Meal preparation
- Linen changes
- Light housekeeping
- Exercises prescribed by your doctor
