HIM is located on the first floor next to the Physician Lounge. Hours of operation are 6 a.m. - 11 p.m., except observed holidays. Weekend assistance is available from 7 a.m. - 4:30 p.m., by contacting the hospital operator (dial 0) and request HIM.

DICTATION INSTRUCTIONS
Wallet-size cards with dictating instructions are available in the HIM Medical Records department or from the Medical Staff Office. Dictating instructions are as follows:
- Dial 844-243-5503 (or 2030 if in-house) to access the dictation system
- Enter your ID number followed by the # key
- Enter the 2 digit worktype
- Enter the Medical Record Number
- Enter the Account/Visit number
- Press 2 to begin dictation
- Press 8 to end and move to next dictation
- Press 5 to end report and end the call

HEALTH INFORMATION MANAGEMENT SUSPENSION OF ADMITTING PRIVILEGES
Delinquent Medical Record is defined as a record which is incomplete more than 30 days from the date of discharge.
Letters are mailed to your office bimonthly with a count of incomplete charts regardless of the number of charts. The letter will list the hospital numbers of all incomplete charts, the names of the patients and items that are incomplete in the chart. These charts must be completed prior to the 30th day after discharge. If the charts are not completed by that time, the following steps will be instituted: Delinquent records are noted with an asterisk*. The physician will lose medical staff privileges if he/she has any delinquent records (unless it is results of system issues and/or not analyzed by HIM properly.) The physician will be allowed to perform pre-scheduled operative procedures and care for patients previously admitted. In addition, if the suspended physician is the primary care physician or the on-call physician for patient in need of admission from the emergency department, he/she will be allowed to admit the patient. If a physician has six or more suspension for delinquent medical records in any twelve month period, that information will be forwarded to the Medical Executive Committee (MEC) for consideration of possible removal from the Medical Staff. If this occurs, it will be reported to the National Practitioner Data Bank. The MEC also ruled that all delinquent charts must be completed prior to the physician having his/her medical staff privileges reinstated or being granted medical staff privileges/membership in the event that the physician loses all medical staff privileges. The Suspension list is posted on ClarkNet at noon, every other Tuesday and can be reviewed at the following link (link to ClarkNet to review list) If you are going out of town on vacation or any other long periods of time, you are encouraged to notify HIM, so you will not be suspended unjustly.
