Orientation Information
If you are interested in applying for privileges to practice at Clark, the first step is to contact the Greater Louisville Medical Society, CAPS department online at www.glms.org or by calling 502-589-2277. In addition to the CAPS application, you will also be required to review materials in our online orientation module and complete a four-hour computer training course.
Completion of each step Clark Memorial Health's provider orientation program is required prior to final approval of privileges and can be completed any time while your medical staff application is in process.
If you have questions or need assistance in completing the orientation program, please contact Medical Staff Services Monday-Friday, 7:30 a.m. – 5:30 p.m. at 812-283-2258. The Medical Staff Office is located on the first floor of the main hospital next to the Providers’ Lounge.
After completion of your online orientation, you will be contacted by the Medical Staff Services department to discuss important next steps, including scheduling your required onsite computer training and a facility tour, should you be interested.
As a new medical staff applicant, you are required to complete this online orientation prior to exercising privileges at a Clark Memorial Health facility. Please make sure to visit all of these sections of the new provider orientation.
Upon completion, a completion notice will also be electronically sent to the Clark Memorial Medical Staff Office to be attached to your application.
