Talk with your doctor about Cardiac Rehabilitation. Phase II requires a physician referral. Our personalized, supportive approach is here for your heart!
Taking recovery to heart by taking charge of your health
Phase II Cardiac Rehabilitation is an outpatient program designed to improve the heart health and quality of life for patients with heart disease. Tailored to meet individual needs, it combines education and exercise in a supportive environment. We also offer Cardiac Rehab Phase III for patients who have completed Phase II or been diagnosed with one or more cardiac risk factors.
Your personal program
Your Phase II program will include supervised exercise, nutritional counseling and education about lifestyle changes that reduce your risk of heart problems. We’ll review your progress throughout the process and share results with your physician.
Exercise is EKG monitored to safely improve heart function, strength and endurance.
Fitness that fits
Patients exercise in supervised sessions using our state-of-the-art fitness equipment. Cardiorespiratory exercises include treadmill, stationary bikes and more. Other exercises include stretching, range of motion and strength training.
Learning for life
The educational element of our program teaches you to understand and manage your heart disease. Cardiovascular risk factor identification and modification give you the tools you need to stay healthy. Your prescribed program can also include comprehensive consultation with a registered dietician. Your consultation will include guidelines for a complete nutrition program created exclusively for you, based upon your nutritional needs.
Our team of experts
Phase II is medically supervised by Advanced Cardiac Life Support (ACLS) certified staff. All of the professionals on our cardiac rehab team are dedicated to helping our patients achieve their goals of improved health and quality of life. We offer the support and encouragement you need during this transitional time in your life.
Best of all, the habits you develop in our program ensure you continue to improve your health long after you have completed your Cardiac Rehabilitation.
We’ll verify your coverage
Most health insurance plans and Medicare cover most or all of the cost of Phase II Cardiac Rehab. We will verify your insurance coverage before you enroll. Phase II Cardiac Rehabilitation requires physician referral
Know Your Risk Factors
Stress, smoking, being overweight and a non-active lifestyle all increase your risk of cardiovascular disease. High cholesterol, high blood pressure, diabetes and a family history of heart disease add to your risk.
